Upon payment of the deposit, my office staff will confirm the receipt of the payment by reaching out to you via phone or email. They will then schedule your appointment and send you the necessary paperwork via email. Please complete your paperwork at least 24 hours prior to your visit.

Dr. Rosenfeldt’s office is downtown in the Pacific Place building next to the Majestic Theatre near Pacific Plaza.
